  • The legal name of the business
  • The business tax ID number
  • The business type (LLC/Sole Proprietor/Corporation/etc.)
  • Business income and liability information
  • Your information, including but not limited to DOB, SSN, address, etc.
  • New business members will need to complete a membership application
